Regardless of where we are (KDP Select/non-KDP camp), there's aways that nagging doubt, am I doing this right?I started with KDP select with my first book in Sep 2018. When I published my second book in Dec 2018, I decided to make my first book wide for few days to test the waters. In a week, I got zero sales outside Amazon, while the ranking for my book nosedived on Amazon.Then I realized that in the pursuit of "not putting all my eggs into one basket", I was putting my eggs in too many baskets! I already have two channels where I create my video courses, and one very active YouTube channel. In addition, I am writing books like crazy. I couldn't focus my marketing efforts on so many channels at the same time.So I decided to come back to KDP Select, and we have had a very happy journey together since.Did I make a mistake coming back? After all, one week is not enough time to reach conclusions whether wide distribution works or not. I don't have answer to that.Will I go wide again? I don't have answer to that either. But one thing is sure, when you think of any alternative, it's better to stick to it, and tell that nagging doubt in your head to sod off!
None of the 20 LGAs met 90% coverage in the proportion of births supervised by skilled birth attendants at baseline and at follow-up (Fig 3A). However, coverage had increased in 8 LGAs at follow-up over baseline; there was no change in 3 LGAs. In another 9 LGA, SBA coverage had actually dropped by the follow-up. In benchmarking the LGA performances with the national and the estimated State coverage, 3 LGAs had performed above the national average of 39% at the baseline and follow up. In another 6 LGAs, performance levels at follow-up fell between at state average of 22.5% and the national coverage of 39% and 11 LGAs fell below the state average coverage. With respect to the coverage of postpartum care for mothers, while it remained poor at baseline and at follow-up, 10 LGAs recorded a higher percent at follow-up in the number of mothers who received postpartum examination within 4 days after birth. Eight LGAs that had zero coverage at baseline had shown some marginal improvements at follow-up (Fig 3B). The percentage of newborns who were breastfed within 8 hours of birth substantially increased from baseline in 15 of the 20 LGAS, reduced in 2 LGAs, and remained unchanged in 3 LGAs (Fig 4A). One LGA (Ganjuwa) already had a follow-up coverage up to the 90% UN target.

Approximately half of under-five deaths worldwide occur in only five countries: India, Nigeria, the Democratic Republic of the Congo (DRC), Pakistan, and China [4]. Nigeria and the DRC fall within the region designated as sub-Saharan Africa. Most deaths in sub-Saharan Africa can be attributed to preventable diseases, including pneumonia, diarrhoea, and malaria, which together killed an estimated 2.2 million children under the age of 5 years in 2012, accounting for a third of all under-five deaths in this region [4]. Children are at greater risk of dying if they are born in rural areas, poor households, or to a mother who has not received a basic education [4]. Early and appropriate vaccination should help to improve outcomes.
A concentration index (CI) provides a measure of socio-economic inequality in health care utilization. Its value varies from -1 to +1: a value close to zero indicates near equality, a value declining towards -1 indicates greater utilization among the poor (pro-poor) while a value increasing to +1 indicates greater utilization amongst wealthier group (pro-rich) . Besides a rich-poor ratio is used to show the inequality in health care utilization among the socio-economic status of the population. Household socio-economic status was classified according to the wealth quintiles constructed by ranking the households using PCA. Five groups (Q1, Q2, Q3, Q4 and Q5) were derived, where Q1 represents the poorest and Q5 the wealthiest. The rich-poor ratio is the ratio between the figures for the Q5 (wealthiest) and Q1 (poorest) for the given indicators of health services utilization.
